Building a Care Guide
What is a care guide?
A care guide provides detailed information to support the recipient's health needs. A care guide can help the caregiver and the person being cared for to record and manage a program of care by providing focused information about the recipient's condition, health history, physician and other medical service provider contacts and preparation for emergency situations.
The following tools can help caregivers and the people they care for to develop care plans which are flexible and can be adapted to meet the individual's needs.
